She may do some custom sized shoes, but a significant portion of the shoes pictured there are mass-produced in China & can be purchased at Milanoo.com & at few other places.

From experience I can confirm that Milanoo do excellent shoes. Both the ones in my avatar and the ones in this picture are from them in size euro 46! The dress came from an auction site seller(lolita4321) who i find very good.Each dress comes with a 2 layer petticoat.
